When we talk about “healthy eating,” the Mediterranean diet is an enduring classic that cannot be overlooked. It is not a strict meal plan, but a traditional dietary lifestyle originating in countries along the Mediterranean coast, such as Greece and southern Italy. For more than half a century, a large body of research has repeatedly shown that long-term adherence to the Mediterranean diet may help reduce the risks of cardiovascular disease, type 2 diabetes, cognitive decline, and certain cancers. It has also been inscribed by UNESCO as Intangible Cultural Heritage of Humanity.
Understanding the core structure as a pyramid
The core structure of the Mediterranean diet can be understood as a “pyramid.” At its base are abundant plant foods for daily consumption—colorful vegetables, fruits, whole grains, legumes, and nuts. Above them is olive oil as the primary source of fat, followed by fish and seafood, recommended at least twice a week. Poultry, eggs, and dairy products are consumed in moderation, while red meat and refined sugar sit at the top and are eaten only occasionally. Adequate water intake and regular physical activity are also considered part of the dietary pattern. Moderate red wine is optional, but people who do not drink are not encouraged to start for this reason.
Fat quality and a high antioxidant load
The health code of this dietary pattern is largely found in its “fat quality” and “high antioxidant load.” In the traditional Mediterranean diet, olive oil is the central source of fat. It is rich in monounsaturated fatty acids and carries abundant phenolic antioxidants, such as oleuropein and hydroxytyrosol. These compounds can reduce oxidative modification of low-density lipoprotein, suppress chronic inflammatory responses, and protect vascular endothelial function. At the same time, abundant plant foods provide dietary fiber, vitamins, minerals, and polyphenols, jointly creating an internal environment favorable to gut microbial balance, metabolic health, and brain protection.
Evidence from PREDIMED and long-term research
Long-term follow-up results from the large clinical PREDIMED study showed that a Mediterranean diet rich in extra-virgin olive oil reduced the incidence of major cardiovascular events by approximately 30% and lowered the risk of type 2 diabetes by more than 50%. In the cognitive field, multiple prospective studies have also observed that greater adherence to the Mediterranean diet is associated with slower cognitive decline and a lower risk of Alzheimer’s disease among older adults. This evidence has helped the Mediterranean diet rank first among the “Best Diets” selected by U.S. News & World Report for many consecutive years.
Starting with every meal
In daily life, beginning a Mediterranean diet is not complicated. Fill half of each plate with vegetables, replace refined staples with whole-grain bread or mixed-bean rice, use extra-virgin olive oil for low-temperature cooking and dressings, eat fish two to three times a week, replace snacks with a small handful of plain nuts, and choose fresh fruit as dessert. Together, these small adjustments create a path toward truly sustainable health maintenance.
